2010 Outdoor Youth Registration Information

It is very important to note that applications will be processed on a first come, first served basis.   Space is very limited due to lack of fields in the area and last year we turned away approximately 400 players.

 

Registration for the 2010 Outdoor Season will take place as follows:

How much will it cost in 2010 if you register by February 6, 2010?

Family Max of $400 per family is applicable if registered by February 06, 2010 but does not include Cobra evaluation fee, volunteer fee or late fee.

(NSF cheques are subject to a $25 administrative fee and must be replaced with a certified cheque or money order within 7 days of notification)

NOTE:  As of February 7, 2010 a $20 per player late fee will apply to all registrations and the family max will not apply.

The registration form is a fillable PDF.  This means you can type the information on the next two pages of this form and then print it with the information intact.

 

BEFORE YOU MAIL YOUR REGISTRATION OR DROP IT OFF AT THE CLUBHOUSE, please ensure the proper forms are included, one complete form per player.  Please print carefully as illegible forms may not be processed immediately.

 

PRINT CLEARLY on all forms. This information is input into two databases and used to contact you throughout the year

Everyone:

Complete the 2010 Outdoor Registration form on the next 2 pages.  Ensure it is fully filled in with the player’s information, parent’s information and signed where applicable.  The second page is mandatory per the Ontario Soccer Association and your registration will not be processed if it is not signed.

NOTE:  If the child is 13 to 17 (inclusive) years old when the registration form is filled out, they must sign the form as well as the parents.

 

Volunteering to be a Coach, Assistant Coach or Manager:

Complete the "House League Coach" or "Cobra Coach "application form.  Please note that the "Police Check" must be fully filled in. Come to the office if you need copies of any of these documents.  (The CUSC will process the “Police Check” free of charge. We will not pay for any Police Checks requested by an individual directly through the Police.)

Volunteering for Other Activities:

Complete your Volunteer Sign-up forms as applicable.  

Include the $35 (per family) Volunteer Fee if:

This is MANDATORY for all families. Your fee will be refunded if you complete an eligible volunteer duty during the year. Please see "Volunteer Info" on the website for complete details.  The applicable volunteer form must be filled out to be eligible for a refund.

Include the $25 (per player) Cobra Evaluation Fee if:

Your child wants to try out for the Cobras. (U9 player Evaluation Fees are included with the Registration Fee). Please check the website under "COBRA Rep Soccer" for evaluation times.

Post Dated Cheques:

Cheques should not be post dated.  If they are your application will only be processed the date of the post dated cheque and your space will not be guaranteed to be still available.  Cheques cannot be post dated for COBRA players.

New Members of CUSC:

If your child is new to the club, include a photo copy of a Birth Certificate, Passport, or other official document clearly showing your child's name and birth date. A Health card is not acceptable for this purpose.

Uniforms:

If the player requires a new uniform, it can be purchased at the Clubhouse. Ensure you check the website under “Office/Equipment Hours” to see when the equipment section is open. Do not leave this to the last minute.

The current price list is as follows for all sizes: (Full new uniform costs $45 tax included)

Shirt - $16 each                     Shorts - $9 each                    Socks - $4 each pair

Note: that both a blue and white shirt is required for children who will be 9 and older.
