Program Guardian Information and Waiver form - (click here)  Please note, this form is a fillable PDF which means you fill in the information online, print the form, sign the form and drop it off at the office within 5 days of registration.  Before you register, please call the office to determine if the program is going to run.  Last year it was cancelled due to only 3 people signing up.
